What Is ECFMG Certification?
The Educational Commission for Foreign Medical Graduates (ECFMG) is the body that certifies IMGs are qualified to enter US graduate medical education. ECFMG certification is mandatory before you can apply through ERAS or register for the NRMP Match.
ECFMG Requirements — The 5 Core Pieces
- USMLE Step 1 — pass.
- USMLE Step 2 CK — pass (3-digit score reported).
- OET Medicine — minimum Grade B in all four subtests.
- Medical school verification — Form 186 + final medical diploma + transcripts via EPIC.
- World Directory listing — your medical school must be listed in the World Directory of Medical Schools as meeting ECFMG eligibility criteria.
Step-by-Step ECFMG Application Process
Step 1 — Apply for USMLE/ECFMG Identification
Visit ecfmg.org, create an Interactive Web Applications (IWA) account, and submit Form 186 with your medical school details. You'll receive your USMLE/ECFMG identification number — the ID that follows you forever.
Step 2 — Apply for USMLE Step 1
Through IWA, submit your USMLE Step 1 application, pay the fee (≈ $1,090 + $220 international surcharge), and pick a 3-month eligibility window. After ECFMG verifies your medical school enrolment, Prometric will let you schedule the exam.
Step 3 — Pass USMLE Step 1 and Step 2 CK

Step 4 — Take OET Medicine
ECFMG accepts OET Medicine (Grade B+ in Listening, Reading, Writing, Speaking). It replaced the old TOEFL requirement and is generally easier for IMGs already working in English-speaking medical environments.
Step 5 — Final Medical Diploma + EPIC Verification
After MBBS graduation, request your medical school to send your final diploma and transcripts directly to ECFMG. Then register with EPIC (Electronic Portfolio of International Credentials) for source verification. EPIC verification can take 4–12 weeks, so do this early.
Step 6 — Issuance of ECFMG Certificate
Once Steps 1, 2 CK, OET, and EPIC are all green, ECFMG issues your Standard ECFMG Certificate — usually within 2–4 weeks.
